This form should be completed by Humanities & Fine Arts faculty members and staff who are requesting a tablet, either as an initial purchase or to replace an aging device.

You will be able to save your information at any time before you click on the "submit" button as long as you have entered your e-mail address.

HFA/University-funded tablets being replaced MUST be returned to Enterprise Desktop Services (EDS) when the new one is deployed.
